  1. The decision to start palliative care in the hospital and transition to hospice care in the community is often the start of the ‘real’ grieving process for the patient and the family. The Palliative Care Team in the hospital has expert members in supporting families and the patient during this transition time. Hospice organizations have nurses and grief experts on staff to help support the patient and the family members outside of the hospital.
